Healthy Mothers and Infants. Maternity Management identifies the risk levels of mothers-to-be and their pregnancies. This information enables us to determine an appropriate level of care and to develop a customized care plan. Our experienced obstetrical nurses offer insightful guidance through one-on-one support and care coordination.
Maternity Management Highlights :
- Education, counseling and health promotion information
- Patient education on the abnormal symptoms of pregnancy and information on behavior modification
- A case manager for the patient to contact if abnormal symptoms appear
- An alert to case managers regarding pregnant patients with abnormal symptoms or risk factors
- A nurse case manager experienced in prenatal care to answer questions up to six weeks after delivery
- Data management and reporting of program activity including outcomes reports
