Health Risk Assessments (HRAs) and Screenings

 

Proven Process.  Dedicated Infrastructure.  Primary PhysicianCare is experienced in delivering Health Risk Assessments to companies and other community groups.  Hospitals and other healthcare providers are invited to utilize our proven process and dedicated infrastructure to provide community outreach wellness programs, or to market health fairs and screenings directly to employers in their community.

 

What We Provide

Scannable HRA Forms

Our HRA form is a one-page OCR scannable form that collects demographics, relevant health information, health history and lifestyle indicators for the member.  Primary PhysicianCare provides scanning and verification services for the HRA forms and imports the data into a secure data warehouse.  HRA forms include data on:

• Member demographics and contact information
• Lifestyle factors such as smoking status
• Current exercise levels
• Dietary questionnaire
• Family history
• Medical history
• Current symptoms
• Current medications
• PCP status
• Screening data

Screening Data

The HRA form provides an area to record screening data and other clinical measures collected at the time of the screening.  Primary PhysicianCare furnishes the Cholestech blood analyzer for immediate results; or the provider may choose to perform standard veinapuncture blood draw for patient screenings. All of this data is imported to our data warehouse. Following are the screening data collected on our HRA forms:

• Height, weight  (to calculate Body Mass Index)
• Blood pressure
• Resting heart rate (pulse)
• LDL, HDL, total cholesterol, ratio
• Triglycerides
• Blood glucose
• HBa1c (optional, with a separate Cholestech analyzer)

HRA Reports

Primary PhysicianCare collects and analyzes the HRA and screening data and provides a comprehensive Health Risk Assessment Report.  The HRA Report includes:

• Executive summary
• Risk levels by disease category such as CAD and diabetes
• Lifestyle trends
• Exercise trends
• Dietary trends
• Stratification of high, moderate, and low risk levels
